How to Apply

To receive an Application Form and a Report Form, select one of the following options:

• Send a request by post to The Duveen Trust
• Fax a request to 020 8349 9649
• Email a request to administrator@theduveentrust.org.uk
• Print both forms from this site

Application Forms must be accompanied by a Report Form from the Organising Agency together with a copy of the programme of the scheme in which you wish to participate and posted to:
